About the Role

Bracknell Forest Council is seeking a compassionate and proactive Leaving Care Personal Adviser to support young people aged 16–25 as they transition from care to independent living. You’ll work closely with care leavers to help them access education, employment, housing, and emotional support—ensuring they’re equipped with the skills and confidence to thrive.

Key Responsibilities

  • Act as a trusted adviser to a caseload of care leavers
  • Conduct holistic assessments and develop tailored pathway plans
  • Support young people with housing, finances, and life skills
  • Promote engagement in education, training, and employment
  • Collaborate with partner agencies and advocate for young people’s needs
  • Organize participation activities and group sessions
  • Maintain accurate case records and work flexibly, including occasional evenings or travel

What You’ll Bring

  • Experience working with young people aged 16–25
  • Knowledge of challenges facing care leavers and unaccompanied minors
  • Strong communication, empathy, and problem-solving skills
  • Ability to work independently and as part of a small, dedicated team
  • Creativity and initiative in developing youth engagement activities
